Scleral Lenses for Aphakia: Vision After Losing the Eye’s Natural Lens

Aphakia is the medical term for an eye that's missing its natural lens — usually after cataract surgery complications, trauma, or certain congenital conditions. These eyes need a very strong correction, and they're often irregular from surgery, which makes ordinary lenses difficult. Scleral lenses can be an excellent solution.

What aphakia means for vision

The eye's natural lens does a lot of focusing work. Without it — and without an implanted replacement lens — the eye is extremely farsighted and can't focus at all on its own. Correction requires a high-power lens, and the eye may also have irregular surface from the surgery or injury that caused the aphakia.

Why correction is challenging

Very high-power glasses for aphakia are thick, heavy, and distort vision at the edges. Standard contact lenses can help but may be uncomfortable or unstable, especially on a post-surgical, irregular eye. Patients often struggle to find a comfortable, clear solution.

How scleral lenses help aphakic eyes

Scleral lenses are well-suited to aphakia because they can:

  • Carry a high correction in a stable, comfortable lens
  • Vault over surgical irregularity for a smooth optical surface and sharp vision
  • Stay centered and stable, avoiding the shifting that plagues other lenses on irregular eyes
  • Hydrate and protect a post-surgical, sometimes dry, ocular surface

Especially valuable for certain patients

Scleral lenses can be particularly helpful for children with aphakia and for adults whose eyes can't accommodate an implanted lens, giving them a path to functional vision that other options can't match.

A precise, individualized fit

Fitting an aphakic eye requires careful measurement and attention to the high power and any irregularity present. Dr. Salley tailors the lens to the eye's unique needs and coordinates with the patient's surgeon when appropriate.
